The Tributes add-on should support more than one tribute type on v3 forms

As an admin, I want to create a new form using the visual form builder. However, for VFB, there is only one tribute type available, and I need at least two: In Honor Of, In Memory Of.

, but the visual form builder supports one tribute type only.

The current workaround is to use v2 forms.

The other part was to be able to have fields for postal addresses (USPS) for the tribute people. e.g., In Honor of John Doe then a field for the email (exists already), then fields for the USPS address so that a future note (hand written) could be sent. It would be like the name and address fields for the VISA card entry but for the Honoree rather than the donor

Thanks for providing this context! Sounds like this small change would go a long way toward building a relationship with your donors. This may not make it into the next few sprints, I'll keep an eye on it.

It is important to have multiple tribute types because our response to the donor may be different given the type. For example, we may want to share our condolences in the event that the tribute is in memory of someone.
